Rate tiers explained: Preferred Plus, Preferred, Standard Plus, and Standard—and what shifts your placement.

Insurers rank term applicants by rate class. Labels shift slightly, but the order holds firm: Preferred Plus (sometimes Super Preferred or Preferred Best), Preferred, Standard Plus, Standard, then tables for elevated risk. Smokers occupy their own tier structure at higher costs.

Height, weight, BP, cholesterol, family heart or cancer before age 60, driving history, medication list, work and pastimes, tobacco or nicotine. Top tiers require clean marks nearly everywhere; Preferred gives some slack; Standard Plus and Standard permit managed conditions like medicated hypertension.
